Some counties may take longer, and others require in-person visits to activate Part B. If you haven't received your card within 6 weeks of submitting an online application, please follow these steps:

Print the 'Employer Verification of Benefits Form' and 'Part B Enrollment Form' below.

Fill them out and get your employer's signature on the verification form.

Take these forms along with your Birth Certificate, ID and SSC to your nearest Social Security Office.
